Below are various forms used to request IT-related services from the CS IT group, including accounts, IP requests, CS Lab software requests, mailing lists, bug reports, etc. Several of the CS IT Support Wiki's pages link back to this page or to specific forms within this page. Some of these forms are print-and-submit, and some are filled out online.
Apply for a CS Linux or Windows Account

Apply for a static CS Network IP Address

This is an online form. Fill out completely, and click Submit.
Request Software to be Installed on the CS Linux Clients.
CS Linux Client Software Install Request Form
This is an online form. Fill out completely, and click Submit.
Apply for a MySQL Account on the CS Dept's Main MySQL Server
CS MySQL Account Request Form in .pdf or .doc file format.
Download this form and print it. When the form is filled out, please submit it to Debbie DeFord in Malone Hall 160 during office hours (posted on the office door).
Request a Mailing List
If you need to run an email mailing list, please fill out the online mailing list request form completely.
The CS department uses Mailman for its mailing lists.
Report a Bug You Experienced on One of the CS Linux Clients
